Dr. Gahramanov grew up in Baku, Azerbaijan, and earned his medical degree with honors from Azerbaijan State Medical University before completing his first neurosurgery residency at the Burdenko Neurosurgical Institute in Moscow, one of the world's oldest neurosurgical institutions.
He then rebuilt his training in the United States: a neuro-oncology fellowship inside Oregon Health & Science University's Blood-Brain Barrier Program under Dr. Edward Neuwelt, a brain tumor fellowship at the Swedish Neuroscience Institute in Seattle, and a second neurosurgery residency at the University of New Mexico. Along the way he authored multiple peer-reviewed publications and presented at national neurosurgical conferences.
After graduating from UNM, he moved to Salem, Oregon, where he practiced for seven years — and in 2020 became the first surgeon in the state of Oregon to perform endoscopic spine surgery. In August 2026, he relocated to Seattle and joined Seattle Spine Institute, where he focuses on the treatment of spine and brain pathologies using minimally invasive and endoscopic techniques, alongside neuromodulation for chronic pain control.
His approach centers on individualized care, using minimally invasive techniques to achieve the best possible outcome for each patient. He treats each patient as a family member, building a treatment plan around that person’s specific needs and goals.
Outside the operating room, he enjoys traveling with family, gardening, fishing, bread baking, and exploring the art of photography.

Procedures
Five areas of specialized practice.
Minimally Invasive Spine Surgery
Small-footprint surgical approaches to spinal pathologies, with reduced tissue disruption and shorter recovery.
Motion-Preservation Spine Surgery
Reconstructive techniques that preserve the spine's natural range of motion, such as cervical and lumbar artificial disc replacement.
Endoscopic Spine Surgery
Camera-guided, ultra-small-incision technique to resolve disc herniation, spinal stenosis, and nerve compression. Dr. Gahramanov performed Oregon's first case in 2020.
Watch the videos →Brain Surgery
Surgical treatment of brain tumors, cysts, hydrocephalus, and other brain conditions. Dr. Gahramanov uses less invasive approaches and the latest technology, including advanced navigation, the operating microscope, and endoscopic techniques.
Spinal Cord Stimulation
Neuromodulation for chronic back, leg, and nerve pain that persists despite other treatment. A small implanted device calms pain signals, and patients try it first during a short trial.
Patient Education
Understanding endoscopic spine surgery
Short videos for patients on how endoscopic spine surgery works — a camera-guided approach through a very small incision to relieve pressure on compressed nerves, typically with less tissue disruption and a quicker return to daily life.

“I cannot express enough gratitude to Dr. Gahramanov for his exceptional skill, care, and dedication. He successfully removed my brain tumor, and quite literally saved my life. From the very beginning, he was confident, compassionate, and thorough, which gave me peace during one of the most frightening times of my life. Dr. Gahramanov is truly outstanding at what he does, his expertise and precision are remarkable. I am forever thankful for the care I received. I also want to thank his incredible staff. Every nurse, assistant, and team member treated me with kindness, professionalism, and genuine concern. I always felt supported and cared for. Thank you all for giving me a second chance at life. I will always be grateful.”
I Could Not Have Put My Brain Into Better Hands!
“From the moment he walked into my hospital room, I felt safe and somehow knew I was going to be okay. Dr. G’s demeanor, direct eye contact, and total attentiveness immediately set the tone and stage for what was simply the best, most clear, most useful consultation I received following the complete surprise discovery of my metastatic melanoma. His follow up with my wife, my after surgery visit, and our discharge assessment at the clinic each displayed Dr. G’s rare gifts of compassion AND complete professionalism.”
“Absolutely awesome. Up and around less than 24 hrs after surgery. […] I put this off for 6 years while exploring alternatives and never got more than 80% relief. Dr. Gahramanov is a dedicated, extremely educated and knowledgeable surgeon. I would highly recommend him.”
Lumbar fusion a fantastic success!
“I had a lumbar fusion almost two years ago, and the surgery was so easy for me, minimal pain afterwards (I went home the same day!) After finishing full recovery with the fusion completely healed (it takes months for the bone to form the fusion) I am golfing, kayaking and jet skiing. I can not emphasize enough how successful the surgery was. Thank you Dr. Gahramanov.”
“Surgery went great, would recommend to anybody who needed neck surgery. Very clean stitches.”
“Very thorough and kind. Fixed my back and no complaints whatsoever.”
“It has been two years since I had my shunt implant and I am doing really well. Dr. Gahramanov did an excellent job. I liked his soft spoken bed side manner and he explained things very well.”
“Dr Gahramanov has been nothing short of exceptional when it comes to my past and present care. Great at explaining procedures as well as what to expect for outcomes and healing. He’s very open to discuss and address all of my concerns and I trust his guidance and opinions. Will not hesitate to use him again should the need arise. Been a pleasure to deal with.”
